АннотацияObjective. To present the experience and technique with hybrid Natural Orifice Transluminal Endoscopic Surgery (NOTES) transvaginal nephrectomy for benign and malignant cases. Methods. 40 patients underwent hybrid NOTES transvaginal nephrectomy for non-functioning renal unit (n = 15) and tumor (n = 25) in 2 academic medical centers. The procedure was performed by the use of multi-instrument ports or flexible cannulas inserted through umbilical and vaginal incisions. The combination of specially designed for the transvaginal access instruments with conventional laparoscopic instruments allowed the standardization of the procedure. Patient demographics, intraoperative and postoperative course were recorded and collected prospectively. Results. Average operative time was113.1 minutes (range 90-190 minutes). Average tumor diameter was 5.9 cm (range 4.9– 7.5 cm). Average umbilical incision length was 2.6 cm (range 2.5–3.5 cm). Estimated blood loss ranged between 50 and 150 mL (average, 80 mL). Average hospital stay was 3.5 days. Complications encountered intraoperatively included 2 bladder perforations. Postoperatively, 2 patients required transfusions due to bleeding and 2 patients suffered from fever of unknown origin. None of the procedures were converted to open or laparoscopic approach. In the cancer patients, no positive surgical margins were detected. No disease recurrence was encountered during the follow up period. Conclusion. The specially designed pre-bent extra-long instruments facilitate the performance of the procedure and allow the standardization of the surgical steps of the transvaginal hybrid NOTES nephrectomy. Hybrid NOTES transvaginal nephrectomy is a feasible and safe alternative to standard laparoscopic nephrectomy for selected patients.
